Ready to Purchase? 
To purchase your ticket follow these simple steps:

1. Complete this Ticket Purchase Form.  Agree to the Dress and Conduct Codes on the ticket form.
2. Fill out the form. Stop at the last question.
3. Pay using PayPal. (Add the name/s and age/s of the ticket holder/s in the note section.)
4. Answer "yes" on the last question of the Google Form and submit it.

That's it! You're all set.

NOTE: The BCHA Winter Formal does not include dinner. We will have hors d'oeuvres and water available.
